Program agreement
Sign and provide this agreement (PDF)  to the Self-Direction Credit program administrator (customers need to sign this only once).

Project application
This is the project application (PDF) to submit a project for review by the Self-Direction program administrator. Submitting an application for pre-approval prior to implementing your project is recommended (but not required). Submitting and application for final approval once your project is complete is required.
