A properly completed Street Light Outage Report Form will help ensure quick and accurate response to your request. Please consider the following tips when completing your form:

Nearest address - e.g.; 123 Main St.

Nearest cross street– e.g.; First Ave.

The six-digit pole number from the pole plate (if you have it).  

Example:

Problem code:
LO = Light out
CL = Cycling light (light is flickering off and on)
DB = Day burner (light is on during the day)
LB = Lens Broken (Glass cover of light is broken)
